Pre-Adoption Screening Form

 

Thank you for choosing to welcome a rescue dog or cat into your family.  You are truly making an incredible difference to an animals life and enabling us to continue saving more animals from pounds and shelters across the state.  We hope this will be your friend for life!

 

Please take a few moments to complete our pre-adoption screening form in as much details as possible to help us ensure the animal chosen a good fit for your home and lifestyle.  Once submitted we will review the application, discuss it with you and arrange a play date with your propsective new furry family member.

 

It is important that your home meets these criteria which are very important to us:

• Your yard is secure and fully fenced to ensure a dog's safety;

• Our dogs must be allowed sleep inside at night, or in a suitable enclosed outdoor space,;

• Cats are to be strictly inside only, unless contained in a secure outdoor cat enclosure;

• As of 2017, we are regrettably not adopting to families with children under the age of 9. Please feel free to ask us about this new safety policy if you have questions or concerns;

• Any children must be supervised when in the company of your new pet at all times;

• If you rent, you must seek and advise us of landlord approval

Completing this form is not an obligation to adopt, but simply gives us basic information on the home, family and lifestyle you have to offer to a rescue animal.  We do not share your personal information with any other organisations and your details can be removed from our database should you decide not to adopt.
